All-on-4 vs All-on-6: what actually differs

Both are the same idea: a fixed bridge anchored to implants, replacing a full arch. The difference is how many anchors carry it — and how much bone you need to place them.

When six implants are genuinely the right call

• The upper jaw. Bone in the upper jaw is softer and less dense than the lower jaw, so extra support is more often justified. This is the single most common legitimate reason.
• Heavy grinding or clenching (bruxism). If you generate a lot of force, spreading it over six anchors rather than four reduces the load on each one.
• A long or wide arch, where four implants would leave a longer unsupported span at the back.
• Good bone volume in six places. This is a real condition, not a detail — six implants are only an option if there is bone to put them in without extensive grafting.

Here is a simple test that catches inflated upgrades. Ask the clinic for both prices — All-on-4 and All-on-6 for the same jaw — then do this:

Is All-on-6 stronger? An honest answer

Zygomatic implants are a genuinely different thing: long implants anchored in the cheekbone, used when the upper jaw has lost too much bone for conventional implants. That is specialist surgery for severe bone loss, not an upgrade to be sold casually — and it should only ever be proposed by a clinician who has scanned you and can explain why the standard options are ruled out.
