The number of grafts you need depends on two things: how much of your scalp needs covering, and how many grafts your donor area can safely give. As a rough guide, one US hair clinic’s planning guideline (Bernstein Medical, New York) puts an early receding hairline (Norwood IIa–III) at about 800–1,600 grafts in a first session, and the most advanced patterns (Norwood VI–VII) at about 3,000–6,400 grafts in total, often over more than one session. Your donor area, not the package price, sets the real limit.

Grafts are not hairs
A graft is a follicular unit: hair in the small, naturally occurring groups it grows in. Modern hair transplants move these units intact, which is what allows large numbers to be transplanted in one session (Bernstein and Rassman, Dermatologic Surgery, 1997).
Each graft holds more than one hair on average. In a 2018 study of ten men, the extracted grafts carried 2.3 hairs on average, with individual patients ranging from about 1.65 to 2.75 (Mohmand and Ahmad, World Journal of Plastic Surgery). At that average, 3,000 grafts is roughly 7,000 hairs. When comparing quotes, check whether a number means grafts or hairs.
Typical graft numbers by Norwood stage
Surgeons usually describe male pattern hair loss with the Norwood scale, a classification first published in 1975 (Norwood, Southern Medical Journal). The table below shows a planning guideline published by Bernstein Medical, a hair restoration clinic in New York. It is a clinic’s guideline, not a peer-reviewed study or an agreed standard. The guideline says the total figures serve “only as a general guideline”, because needs vary with hair characteristics, density, scalp laxity, head size and shape.
| Norwood stage | What it looks like | First session | Total, one or more sessions |
|---|---|---|---|
| IIa | Recession at the temples and front hairline | 800–1,400 | 1,400–2,200 |
| III | Deeper recession at the temples | 1,000–1,600 | 1,600–2,400 |
| III vertex | Temple recession plus thinning at the crown | 1,200–1,600 with crown: 1,600–2,200 |
1,800–2,600 with crown: 2,600–3,200 |
| IV | Front and crown thinning, still separated by a band of hair | 1,600–2,200 with crown: 2,200–2,600 |
2,200–3,400 with crown: 3,400–3,800 |
| V | Larger front and crown areas, the band between them narrowing | 2,000–2,500 with crown: 2,500–2,800 |
2,600–3,800 with crown: 3,800–4,500 |
| VI | The band is gone; front and crown have merged | 2,400–3,000 with crown: 2,800–3,400 |
3,000–4,600 with crown: 4,600–5,600 |
| VII | Only a horseshoe of hair remains at the sides and back | 2,500–3,200 with crown: 3,000–3,600 |
3,200–5,000 with crown: 5,000–6,400 |
Other sources give higher numbers. In a 2020 paper in the Journal of Cutaneous and Aesthetic Surgery, one surgical team planned about 2,500–3,000 grafts for Norwood IV, 4,000–5,000 for Norwood V, 6,000–7,000 for Norwood VI and 7,000–8,000 for Norwood VII, but it reached those totals by adding beard and body hair to scalp grafts, often over several sessions (Garg and Garg, 2020). There is no standard graft number for each stage, so treat any chart as a starting point for a conversation with a doctor.
The Bernstein guideline advises including the crown in a first session only with an above-average donor supply and limited expected further loss.
Your donor area sets the limit
Grafts are taken from the back and sides of the scalp, where hair is more resistant to male pattern hair loss. The ISHRS puts the problem plainly: “Patients have a supply of donor hair that is finite and it’s a non-renewable resource.” It adds that a surgeon “cannot replace each hair that has been lost on a one to one ratio because that amount of donor hair does not exist” (ISHRS).
How much you can safely give depends on you:
- Donor density. In the 2018 study above, donor density averaged about 155 hairs per cm², and the authors recommend that FUE removes less than 35% of donor density in a first session and no more than 10–20% in a second (Mohmand and Ahmad).
- Family history and age. The ISHRS notes that a family pattern of severe hair loss may mean the “safe” donor area is limited, and that younger men with progressive loss “may need several transplantation procedures over a number of years” (ISHRS).
For a sense of scale, the 2020 paper estimated the safe scalp donor area at about 12,500 grafts, of which about 6,000 can be safely harvested, and used body hair to make up the difference for Norwood VII (Garg and Garg). That is one team’s calculation, not a rule, but it shows why a lifetime plan matters more than one big session.
How many grafts can be done in one session?
There is no universally agreed maximum. In the Bernstein guideline, first sessions range from about 800 grafts for early recession to about 3,600 for Norwood VII with the crown. In the 2020 paper, a first sitting of about 5,000 grafts for Norwood VI ran over two consecutive days, with about 2,500–3,000 taken from the scalp and the rest from the beard or body (Garg and Garg).
Going too far has a visible cost. The ISHRS patient-safety campaign shows a case captioned “4500 Grafts Harvested in a Single Session. This is an exhausted donor”, and describes FUE overharvesting as the result of taking too many follicular units from one area and an “overaggressive attempt to excise a lot of grafts in a short amount of time” (ISHRS).
Why “unlimited grafts” deserves scepticism
“Unlimited grafts” describes how a package is priced, not how much hair you can have. On our hair transplant in Turkey page we describe it as the maximum your donor area safely allows in one session, and that maximum is set by your scalp, not the package. A plan that uses every available graft at once can leave nothing for later loss; the NHS notes that your hair might continue to thin around the transplant (NHS). For what a package usually covers, from hotel nights to aftercare, see our guide to what is included in a hair transplant package in Turkey.
How graft count affects the price in Turkey
Turkish clinics quote either a fixed package or a price by graft band, so ask which applies to you and what happens if more grafts are used. As planning figures:
- All-inclusive FUE packages start from around €1,200.
- We do not publish planning figures for Sapphire FUE, DHI or graft bands (see our Turkey price table); ask the clinic to price each option in the same written quote.
- In Istanbul, typical FUE quotes are around €1,500–3,000 (Istanbul prices); €1,200 is the lowest planning price, not the typical quote.
A plan above 5,000 grafts usually means advanced loss and often more than one session, so get it priced individually. For comparison, the NHS says a hair transplant in the UK can cost between £1,000 and £30,000, depending on the extent of hair loss (NHS).
Which photos to send for an online assessment
A clinic can give you a provisional graft range from photos, but the final number should be set by a doctor who examines your scalp in person. Turkish rules require clinics to photograph the donor and recipient areas before the procedure and to record the technique and the number and location of grafts implanted (Regulation on Hair Transplant Units, Article 10). Good photos at home make the first estimate more useful. Take them in daylight, with dry hair and no styling products or hair fibres:
- Front: straight on, with your hair pushed back to show the hairline.
- Both temples: one from each side, at an angle, to show recession.
- Top: from above, to show the mid-scalp.
- Crown: from above and behind; ask someone to help.
- Back and sides (donor area): close enough to show density.
Add your age, how long you have been losing hair, family history, current medication and any previous transplant. Then ask who reviewed your photos and how many grafts the plan leaves in reserve. The NHS notes that hair transplants suit permanent, inherited baldness and are not generally suitable for other types of hair loss (NHS). You can also check a clinic against the Ministry list.
Frequently Asked Questions
How many grafts do I need for Norwood 3?
As a rough guide, a US hair clinic’s planning guideline (Bernstein Medical) suggests about 1,000 to 1,600 grafts for a first session at Norwood III, and about 1,600 to 2,400 grafts in total over one or more sessions. Your own number depends on your donor density, hair characteristics and the area you want covered, so treat any figure as an estimate until a doctor has examined you.
Is 6,000 grafts in one session realistic?
Rarely, and it deserves questions. In a US hair clinic’s planning guideline (Bernstein Medical), first sessions top out at about 3,600 grafts, and 5,000 to 6,400 grafts is the total for the most advanced pattern over one or more sessions. One 2020 paper estimated that about 6,000 grafts can be safely harvested from the scalp in total, by one team’s calculation. Ask how many grafts would come from the scalp, over how many days, and what that leaves for the future.
How many hairs are in one graft?
A graft is a follicular unit, a small natural group of hairs. In a 2018 study of ten men, the extracted grafts carried 2.3 hairs on average, with individual patients ranging from about 1.65 to 2.75. So 3,000 grafts is roughly 7,000 hairs, not 3,000.
How much do 3,000 or 5,000 grafts cost in Turkey?
Turkish clinics quote either a fixed package or a price by graft band, so ask which applies to you. All-inclusive FUE packages start from around €1,200 in our planning figures; larger graft counts, Sapphire FUE and DHI are priced by the clinic. The final quote depends on your assessment and the clinic.
Can I have a second hair transplant later?
Often, if your donor area allows it. The ISHRS describes donor hair as finite and non-renewable, and advises treating any transplant as your first. A second session is usually considered only after the first has fully grown in, which can take around a year.
